Greater Western Water (GWW) creates audio, video, and photographic content (collectively referred to as materials) featuring participants for various purposes including marketing, social media, and promotional activities.
These materials may be reproduced by GWW in several formats, including:
By agreeing to this form, you consent to GWW using and storing your image, name and likeness for the above purposes, and acknowledge that:
1. Permission
I grant GWW permission to use my image, likeness, and voice in photos, videos, and other media for marketing, social media, and promotional materials.
2. No compensation
I understand that I will not receive any payment for the use of my image.
3. Withdrawal
I can withdraw my consent at any time by contacting marketing@gww.com.au
4. Release of liability
I release GWW from liability related to the use of my image.
E.g. tree planting day, site tour.
E.g. green jumper, curly blonde hair, red glasses.

The personal information requested on this form is being collected by Greater Western Water in accordance with the Privacy and Data Protection Act 2014 (Vic), for the purpose of obtaining your consent for its use in the project described above, or any other directly related purpose.
