Use this dispensation form to request a departure from compliance with Greater Western Water's applicable design standards.
A design dispensation request is to be submitted by the key design personnel (checker).
Please include a Greater Western Water Reference Number for Land Development (LND number) where applicable.
The organisation name and contact name should be as they appear on the accredited contractors or consultants list.
All fields are required unless marked as ‘optional’ or ‘if applicable’.
